Months 4 to 6: the steady phase

Most patients are at or near their effective dose by now, and the trend is steadier and more predictable. Metabolic markers frequently improve — blood glucose, blood pressure, triglycerides and liver markers.

Many patients report the improvements they value most are not on the scale: sleep quality, joint pain, mobility and energy.

Months 6 to 12: approaching plateau

Weight loss slows as the body adapts. This is expected physiology, not failure. Reviews shift towards protecting the result and maintaining muscle.

Why results vary so widely

What the scale does not measure

Body composition matters more than total weight. Two patients losing the same number of kilograms can have very different outcomes depending on how much was fat and how much was muscle.

Waist measurement, how clothes fit, energy levels, sleep quality and blood markers together give a far more accurate picture of progress than weight alone.

Plateaus and what they mean

A plateau after several months is normal physiology. As body mass falls, energy requirements fall with it, and the body defends against further loss through hormonal adaptation.

Plateaus are usually addressed by reviewing protein intake, resistance training, sleep and dose rather than by dramatic calorie restriction, which tends to accelerate muscle loss.
