Why you start low

The starting dose is not intended to produce weight loss. It exists to let your digestive system adapt to slower gastric emptying before the appetite effect is strengthened.

Beginning at a higher dose does not accelerate results. It reliably produces nausea severe enough that many patients stop altogether.

How the ladder progresses

You remain at each dose level for a defined period, commonly around four weeks, before your doctor considers an increase. Progression is never automatic.

At each review your team assesses weight trend, side effect tolerance, nutritional intake and any new symptoms. If you are struggling, the dose holds rather than rises.

Not everyone reaches the top dose

Some patients respond well at a middle dose and stay there. Others need the maximum. Neither is better; the right dose is the lowest one achieving a good result you can tolerate.

Since higher doses cost more, stopping at an effective middle dose is often the sensible outcome rather than a compromise.

Timing and technique

The injection is weekly, on the same day each week, given into the fat layer of the abdomen, thigh or upper arm. Rotate the site to avoid irritation.

It can be taken with or without food, at any time of day. Choose a day that fits your routine and keep it consistent.

If you miss a dose

Follow the specific guidance your doctor gave you when prescribing rather than guessing or doubling up. Doubling to catch up causes significant side effects.

If several weeks have been missed, contact your team before restarting. You may need to step back down the ladder rather than resume at your previous dose.

Holding or reducing the dose

A dose hold is a normal clinical tool, not a setback. If side effects are difficult, staying at the current level or stepping back down usually resolves it while keeping you on treatment.

Never adjust the dose yourself. Escalation and reduction are both medical decisions.

Injection technique that reduces discomfort

Allow the pen to sit at room temperature for a short period before injecting; cold medicine stings more. Clean the site, pinch the skin gently, and inject at the angle you were shown.

Rotate sites systematically rather than using the same spot weekly, which prevents lumps and irritation. Dispose of needles in a sharps container, never in household waste.

What to record between reviews

Weekly weight at the same time under the same conditions. Waist measurement fortnightly. Any side effects with the day they occurred relative to your injection.

This turns a vague report into useful clinical information. Patients who track well get better-tailored dose decisions.

When escalation should pause

If you are still experiencing significant nausea, vomiting or reduced intake at the current dose, increasing is usually the wrong move. So is escalating during an acute illness.

A pause is not lost progress. Most patients who hold a dose for an extra few weeks go on to tolerate the next step far better.
